CALL FOR SESSION PROPOSALSThe conference planning committee seeks session proposals reflecting the conference theme and focus.
Target Audience The conference convenes clinical, public health, healthcare, community-based organizations, and social services professionals to build bridges, understand each other's perspectives and learn best practices in cross-sector partnership and teamwork. Conference Breakout Sessions We invite session proposals on key health equity levers of change including: Culturally and linguistically appropriate services; data practices to support the advancement of health equity; workforce engagement; community collaboration; and public health policies to improve population health and advance health equity. Session proposals can focus on innovative public health or clinical approaches, key strategies, leading practices, and building alliances and cross-sector collaboration to improve health outcomes, patient care, identify inequities in health care provision, and to implement effective interventions to improve client-centered services or patient care services. Breakout Session Format The conference breakout sessions are scheduled concurrently. Each session is scheduled for one hour, including 5 minutes of introduction, 45 minutes of interactive panel discussion, and 10 minutes for questions and answers. Proposal Evaluation Session proposals will be evaluated, and the final selection will be made by the conference planning committee. If selected, speakers will receive discounted registration rate.
