2023 TEAM UP TAKE ACTION CONFERENCE

In partnership with Dartmouth Health and Vermont Public Health Association,  NHPHA invites you to join us at the 2023 Team Up Take Action Conference on Friday, December 1, 2023 at the Hanover Inn Dartmouth in Hanover, NH. ​This year's conference theme is Improving Health Equity: Programs, Policy, Measurements, and Workforce. 

BACKGROUND
The Team Up Take Action is an annual health conference that began in 2016 through a partnership between Dartmouth-Hitchcock Medical Center, Cheshire Medical Center, and the New Hampshire Public Health Association. The conference was developed in response to a growing recognition that to truly improve health equity, an interdisciplinary, coordinated, cross-sector approach is necessary.

​The conference aims to engage the public health, healthcare, and social services workforce to build bridges, understand each other’s perspectives, and access education and training in cross-sector partnership and teamwork to address health disparities and improve health equity. The Vermont Public Health Association joined the partnership in 2017, extending the audience to both sides of the Connecticut River. Conference partners believe that to advance health equity and improve health outcomes, hospitals, and health systems can no longer view themselves as separate from public health and social service sectors.​

TARGET AUDIENCE
Team Up Take Action convenes public health, healthcare, and human and social service professionals to build bridges, understand each other's perspectives and learn best practices in cross-sector partnership and teamwork. The audience is professionals in public health, healthcare system, social services, community-based organizations, and students preparing for careers in all sectors.
