TEAM UP TAKE ACTION CONFERENCE

In partnership with Dartmouth Health, Vermont PHA, and Maine PHA

DECEMBER 9, 2022   I   Virtual Meeting

CALL FOR PRESENTATIONS

The conference planning committee seeks presentation proposals reflecting our theme and focus. We’ll explore creative new alliances and approaches that have emerged from the pandemic and delve into ways to increase our effectiveness as we work to ensure health equity to transform communities, so they are more sustainable and just.

Conference Overview

Target Audience
The conference convenes clinical, public health, healthcare, community-based organizations, and social services professionals to build bridges, understand each other's perspectives and learn best practices in cross-sector partnership and teamwork.

Conference Tracks
We invite presentation proposals to the two tracks shaping this year’s conference. Use these tracks to guide your submissions and choose the one track that most closely aligns with your proposal.

Track I – Building Alliances and Cross-Sector Collaboration between Public Health and the Health System to Promote Health Equity
This track consists of four breakout sessions. Session proposals can focus on one or two of these areas:
  • Alliances or cross-sector collaboration where organizations were able to create healthy and equitable communities.
  • Community-based interventions designed to improve population-based outcomes.

Track II – Innovative Approaches to Clinical Health/Public Health to Promote Health Equity
This track consists of four breakout sessions. Session proposals can focus on one or two of these areas:
  • Innovative clinical approaches which demonstrate the capacity to improve public health outcomes.
  • Innovative public health approaches which demonstrate the capacity to improve clinical outcomes.
Breakout Session Format
The conference breakout sessions are held virtually and are scheduled concurrently. Each session is scheduled for one hour, including 5 minutes of introduction, 45 minutes of presentation or panel discussion, and 10 minutes for questions and answers.

Proposal Evaluation

Session proposals will be evaluated, and the final selection will be made by the conference planning committee. If selected, speakers will be able to attend the day of their presentation at no cost and will receive a discounted rate if they decide to attend the whole conference.

If you have any questions regarding the submission process, please contact Hanan Bedri, NHPHA Executive Director at hbedri@nhpha.org.
